In order to begin registering your child, please complete the online pre-registration questionnaire.  There are documents that will need to be filled out and submitted separately before your registration is complete.  

All additional forms/information may be emailed to kristinwarren@bloomfieldcsd.org.  You may also drop the documentation off at the District Office (45 Maple Ave, back of the elementary building by the Intermediate playground) between 8 am - 3:30pm
