Welcome to the Wyoming Central School District New Student Pre-Registration Process ------------------------------------------------------------------------------------- It is our goal to make this transition as quick and positive as possible. After you have completed the Pre-Registration, we will contact you to schedule a Registration Appointment. It is at that time we will need you to bring the following: - Completed Student Information Sheet (click here for link: Student Information Sheet)
- Child's Birth Certificate or Passport
- Proof of Residency (lease/deed, rental contract, utility bill, letter from landlord, letter from person you are living with, other)
- Proof of Custody or Guardianship (if applicable) or Foster Care Letter
- Immunizations Records (See NYS Guidelines attached)
- Last Physical (must have taken place within the past 12 months)
Additional educational documents (If applicable): - Latest report card from previous school/preschool
- IEP
- 504
All registrations will be considered incomplete until all documentation listed has been provided.
